Forms for Student Athletes
The Kansas State High School Activities Association (KSHSAA) must have physicals on file before any student athlete can participate in any sport. They cannot try out without a doctor-approved physical form.
Athletic physicals for the 2016-17 school year must be dated after May 1, 2016.
The doctors with SAFEKC and OrthoKC are hosting a sports physical clinic for all Bishop Ward students who plan on participating in ANY sport for the 2016-2017 year.

This is the ONLY clinic available to BWHS students for the Fall.
The cost is $20 per student.
Parent attendance is STRONGLY recommended. A parent-signed physical form and concussion form are REQUIRED if the parent cannot attend.
Student athletes can receive a quality physical exam given by the sports medicine physicians of OrthoKC. The certified TeamWorks athletic trainers will be available for questions, too. This comprehensive sports physical also includes an EKG and an evaluation by a board-certified cardiologist from Providence Medical Center.
